LHSC Launches Innovative Home Care Program for Patient Recovery
Patients transitioning from hospital to home often face significant challenges, particularly after extended stays. The London Health Sciences Centre (LHSC) has introduced the LHSC To Home program to provide essential support for these individuals. This initiative aims to ease the recovery process by offering personalized home care services, making a crucial difference in patients’ lives.
One of the first beneficiaries of this program, Elisabeth Hutchison, reflects on her journey. After being admitted to LHSC in March 2024 for various medical issues, she spent nearly a year in care before returning home. “I’m a work in progress. I’m getting better,” said Hutchison. “I was so happy, but also very much weaker than I anticipated.”
Transitioning home is often more complex than anticipated. Hutchison noted the drastic change from having regular meals and constant nursing care in a hospital to managing those needs independently at home. “You take for granted that you have three meals a day, you have nurses to look after you,” she explained.
The importance of the LHSC To Home program extends beyond physical health. Hutchison’s daughter, Sarah Hutchison, emphasized the significant impact of being home on her mother’s mental well-being. “She so needed to be home as part of her recovery. I think it was really, incredibly important for her own mental health,” Sarah stated.
The LHSC has partnered with SE Health, a non-profit social enterprise health provider, to implement this program. Supported by provincial funding, the initiative offers up to 16 weeks of personalized home care. According to Shelby Fisch, Vice President of the Acute Transition Program for SE Health, the program is tailored to each patient’s specific needs. “Our primary nurse goes in, assesses the patient, and we bring in the whole interprofessional team, which includes physiotherapists, occupational therapists, personal support workers, social workers, and dietitians,” Fisch explained.
In its inaugural year, the LHSC To Home program has successfully assisted 500 patients. This has not only provided essential care but also alleviated pressure on hospital resources. Tammy Quigley, Vice President of Cancer, Renal, Mental Health, and Patient Flow at LHSC, highlighted the broader impact of the program. “Our emergency department wait times can be decreased. It means that ambulances aren’t waiting to offload patients into the emergency department. It means we’re not canceling surgery. It is an absolute game-changer for us,” Quigley remarked.
Looking ahead, LHSC plans to expand the program, aiming to support an additional 100 patients in the coming year. For the Hutchison family, the transition home has been a vital step in the recovery process. Elisabeth expressed her gratitude for the program, stating, “My family really turned up for me. But it was nice to be able to get that extra help.”
As healthcare systems worldwide grapple with the challenges of patient care and resource management, initiatives like the LHSC To Home program serve as a promising model. This approach not only supports patients in their recovery but also optimizes hospital capacity, ultimately enhancing the quality of care provided to all.
